An adverse drug reaction (ADR) is an unintended, harmful reaction to a medicine taken at a normal dose. If you or someone you know has experienced an adverse reaction to a medicine, please report it using one of the methods below.

What to include in your report:

  • Patient details (age, sex, weight)
  • The suspected medicine (name, dose, frequency)
  • Description of the adverse reaction
  • Outcome of the reaction
  • Any other medicines being taken
